The Sixty Day to Nine Month Assessment Program (SNAP)

The Sixty Day to Nine Month Assessment Program (SNAP) provides assessment and a step by step plan for treatment and habilitation for use within the home and community school settings.

Students enrolled in SNAP will receive a complete assessment by an interdisciplinary team including special educators, behavior specialists, occupational therapists, speech and language pathologists, case managers, and medical consultants. Chileda's interdisciplinary team has an average of eighteen years experience in the field of special needs and program development.

The assessment phase of SNAP will be individualized but generally include a functional behavioral analysis, educational achievement levels, sensory evaluation, speech and language assessment, medication review, and any other warranted evaluations. Based on the assessments, observations, and program trials at Chileda an individual plan will be developed. the student's plan may be integrated into the child's IEP. Upon discharge from SNAP families and schools will be provided specific treatment approaches, guidelines for assistive technology, sensory and environmental control options, communication systems, and a practical step by step plan for implementation and monitoring of the program recommendations.

As part of SNAP family members and teachers will be invited to participate in a flexible schedule of inservice trainings on the Chileda Campus. These inservices will provide a foundation for understanding treatment modalities, the rationale for specific program components, and the skills for implementation and monitoring program recommendations. Inservices offered include TEACCH Training, Communication Strategies, Writing Social Stories, Crisis Prevention Intervention, An Effective IEP, Behavior Strategies Proactive Tool Kit, Coping Skills Training, Social Skills Training, Theory of Mind/Social Implications, Sensory Integration, and others.
